It is with our greatest regret to inform you that we will be discontinuing our Intelliform service and will not be accepting any new registrations. To aid in your transition, we will keep the server open so that you can access your account and print out your survey responses (or save as PDF).
We are grateful to have been able to serve you during this time and we wish everyone health and safety as we continue to navigate through this pandemic.

How do I get started?
Unfortunately, we are no longer taking new registrations. However, existing users will still have access to their accounts to retrieve all their data. Click here to log in.
Is there a User Guide?
What is Intelliform for?
You can screen customers and fill in the forms as you ask your customers the questions. The responses will then be saved to your account and can be accessed again when needed.
How do I get support for Intelliform?
Logic Tech Corp is unable to provide any email or telephone support. However, feel free to visit our forum if you should have any questions. This will be monitored by our administrators.
